A registered charity in Hamilton, Ontario, offering presence, dignity, and time for our communities as we navigate the contemplation of life and death. Journey Home Support walks alongside individuals, families, and caregivers who are navigating a serious illness, a palliative or terminal diagnosis, or seeking to live and die with greater awareness.


We are with the intention to bridge the gap between clinical care and the emotional, psychosocial, and spiritual support which many are missing and seeking, especially as death and dying come onto the journey home.

Founded by psychotherapist and end of life doula Renee Moor, our organization exists to ensure respect, dignity, connection, peace, and trust at end of life, through grief and empowered living.

We are different from hospitals and hospices because our focus is on non-clinical companionship and support in people’s homes, surrounding individuals and families with a calm, attentive presence rather than monitoring equipment and rushing through schedules.


We serve Greater Hamilton and surrounding communities, providing our core supports free of cost so finances are never a barrier to compassionate care
